In a continuous effort to improve service for our patients, and in an effort to help save you time,
the Marietta Eye Clinic now offers patient paperwork for download or direct registration via e-mail.
The following documents open in Adobe PDF or can be completed and submitted.
If you do not have Adobe PDF, you may download it for free from Adobe by visiting their website,
adobe.com.
Please make sure you are using the most current version of Adobe PDF for these files to open!
Please download, print, and complete the documents or complete them on-line and submit. If you decide
to print the forms, please bring them with you to your scheduled appointment, and spend less time waiting.
You will not be able to save the documents to your computer. Please complete them on your computer and
print a completed copy.
Problems with the Patient Visit Forms?

- Make sure you are not using a firewall that prohibits downloading documents.
- If you cannot get the forms to work, please email the webmaster with the specifics of the problem.
Include information about the form you are downloading and the specific nature of the problem as well as
troubleshooting you have already tried.
