We have implemented an electronic patient registration system to reduce the amount of paperwork that patients need to complete when checking-in for their appointment. When arriving to the office, you will be directed to one of our kiosks to confirm and update your demographic and insurance information. Please be prepared to scan in your driver's license and insurance card(s). You will also have the opportunity to electronically sign forms, as needed. The only paperwork you may need to complete is a medical history form, if you are a new patient, or if it has been more than 12 months since your last appointment. You are welcome to print the form and bring it into the office with you. Once you have been enrolled in the EMA patient portal, you will also have the ability to complete that information electronically in a secure manner.
Medical Record Release
Obtaining a copy of your Marietta Eye Clinic Medical Record is easy. To start your request, simply download, print, complete and sign the following forms and fax or mail them back to us using the contact information at the top of the form. You may also send it via email to 9_Medical_Records@mariettaeye.com. Please send to attention "Release of Information, (Medical Records)." Before sending, make sure you have completed and signed the form. Unsigned and incomplete requests cannot be processed.
Your request will be processed and fulfilled within 30 working days. We will either mail your copy to the address specified on the following authorization form, or you may pick up your copy from our offices located at the address above between 8 a.m. and 4:30 p.m., Monday through Friday.
Copies are $0.97 cents per page. We will send you an invoice by mail to let you know how much the total charge will be once we retrieve your records. You may pay by debit/credit card or send a check by mail or in person should you wish to retrieve your records personally.

PLEASE NOTE: Marietta Eye Clinic has transitioned to a new electronic health record system (EHR). As part of this transition, our patient portal has changed. If you are registered with NextGen's NextMD Patient Portal, you will be able to view information from past visits there but all other functionality has been disabled. Information for all visits moving forward will be accessible through our new EMA Patient Portal.
The Marietta Eye Clinic EMA Patient Portal allows electronic communication between you and your providers at MEC. This internet-based patient portal is a secure, confidential and easy-to-use website giving patients 24-hour access to review health information sent from your physician's office. Secure messages and information can only be viewed by those individuals who enter the correct username and password to log-in to the Marietta Eye Clinic Patient Portal site. By signing up and enrolling in the patient portal, you will be able to:
- Send and receive secure messages to and from your provider
- Complete medical history, in advance of your visit
- View visit summaries from your appointments
- Obtain health-related educational materials
- Update account information
